PROBLEM/ABSTRACT:
Promoting elements from a workspace and associating those changes to an issue reports the following message:

For one or more of the versions to be promoted, an entry already exists in the specified change package. Moreover, the version to be promoted need to be merged with the version in the existing entry.
Click Cancel to cancel the Promote command now.
Click Proceed to proceed with the Promote command. It will succeed only if the changes to be promoted already include the current change package entry.
In either case, the change package will not be modified.

Clicking on Proceed reports the following warning:

Promoting elements.
The version to be promoted does not contain all the changes in the specified issue record: <pathname to element>

SOLUTION:
The reason why you are getting this error is due to the real version in which you are trying to promote and associate against this issue is not a direct descendant to the head version in the change package.
You have two options in which you will be able to associate this element with this issue in which you are promoting against.
1. Open the issue which you are trying to promote against -> click on changes tab -> select element -> click on red x to remove from issue.
Now promote the element and associate to this issue.
2. Determine the current head version of the element in the issue you are trying to promote against -> Open the browse version window for this element from your workspace which you are promoting from -> Select the head version -> right click -> merge from.
Now promote the element and associate to this issue.
