This error is caused by a known issue with the base-build of the StarTeam 12.0 Server (build 188.8.131.52). It occurs when the Server has an Enterprise license.
The upgrade process is attempting to upgrade the "Requirements" section of StarTeam and as this feature is only available to Enterprise Advantage licenses, the database information for 'Requirements' does not exist and this causes the upgrade to fail.
StarTeam 12 Server build 184.108.40.206 Patch #1 has been released to address this issue.
