This error can occur if you have just recently upgraded from CaliberRM 2008 SP1 or 10.1 to Caliber 11 and your requirements include traces to Quality Center items.

To resolve the problem you will need to register the Quality Center (QCintegrationAddin.dll) file on each of the client installations.
