And here we see the timeout values of the processes which should have timed out (but never does)

Do you have to add a line for the Approval to follow on Timeout?  Click on the Approved line, look at properties, and see if the drop down that says Approved, Rejected includes Timeout. I forget offhand.  If so, you can add another line to tell it what to do WHEN it timesout.

Forgot to say that I have tried that too.

Anyway, It actually helped to restart idapps.

And now that we are on that track. It is an AWS based docker installation, and a couple of days ago the SaaS database complained about lack of possible connections, and it was a bit slow to scale up. Perhaps idapps choked on that.
