ANSWER:
The file is called com.accurev.eclipse.zip and is located under the <AccuRev install>\bin directory.

Using a tool that can process ZIP files, unpack com.accurev.eclipse.zip to the Eclipse plugins directory. Directory com.accurev.eclipse_x.x.x must be a sibling of all the other Eclipse plugins. The directory hierarchy should look like this:
