From the last screen shot I see that settings contains incorrect URL: port 8080 shouldn't be specified in the connection string.
Could you please specify following connection string, reopen Visual Studio Code and check requests list:

dimensions://dim12test/kddimdb@dim12/MTI

Could you please provide additional information for following questions:
1. What the version of Dimensions CM server are you using?
2. What the version of Git Client are you using? Note: Can be checked by running command "git dm version" in cmd. 
3. Could you please clarify if following command in cmd works fine:
git dm config dimensions://dim12test/kddimdb@dim12/MTI DM_WEB_URL

Could you please collect logs?

For that close all opened projects, set Debug log level in Dimensions extension settings.

Open View->Output, and select Dimensions channel in combobox.

Open Request view, and collect logs.

From the first log I see that debug log level is not activated.
Also I see from the log, that looks like some project is opened now in VSCode. Did you perform clone of stream YMA in VSCode?

Request view loaded requests in context of opened stream. If nothing opened then URL from settings will be used.
Could you please close folder (File->Close Folder) and try refresh request view to load requests from URL specified in the settings?
And please provide logs again.

From the second log (where error "Must provider a valid ScmUri" is mentioned): Command Prompt overlaps log where URL is mentioned, could you please provide full log?
