Two files with the same name in the same folder

Has anyone ever seen this happen? I am trying to piece together the "how" and "why" of this issue. We have a project that contains two copies of the same file- with the exact same name and extension. One indicates it has been branched, and the other is not branched. I'm thoroughly confused as to how StarTeam even allowed someone to check in two files with the same name.
