Workflow is executing and updating the column, but not showing the user action in my workflow tasks

Dear Experts,

I have created a workflow to update the columns based on the inputs and assigning the user action after making the changes in status.

status column is updated and it's not creating my workflow task assigned to user. need your inputs at earliest.

Below is example, Any changes happened on the column Application Name, Validation status should change to "Pending Approval" and launch the wizard for user to take action.

but status changes happening but my workflow tasks are not creating.

    Your workflow requires a user-action type workflow activity in which case the assigned user or group accesses via "My workflow tasks" to launch the wizard.  Wizards do not auto-start (ie. launch) from a workflow.

    it is having the user action and a wizard is assigned to it too.

    but still its not launching.

    any suggestions.

    Contact MF Support as I cannot help without seeing how the workflow is setup.  I assume "...but still its not launching..." means assigned user starts the wizard manually from the "My workflow tasks" but nothing actually happens.  I assume the wizard has same context as workflow and the context record has not changed.
