Chat SD - add a "Service Recipient" field to the default SD screen for help desk.

When a user starts chat from the portal - the help desk is presented with an SD screen.  That screen (OOB) does not have the same fields as the create SD from scratch does.  Especially notable is the missing "Service Recipient" field.  This can be used when some people initiate the chat with the help desk for others (e.g. Admin for an Executive).
