Organizing fields in a form

Hello, we would like to be able to organize the logical fields and the user option, in the forms, as we want :

Exemple :

Logical field 1 - User Option 1

Logical field 2 - Logical field 3

User Option 2

Logical field 3

    Please describe more about the use case scenario - is this in the self-service portal, or for the agent? And/or provide an example of where the logical field and user option need to be laid out in that way.
