Currently there is no chance to customize the Service Portal pre-chat panel using standard feature in Service Manager (Service Desk ==> Administration ==> Configure pre-chat).

In the OOB only two fields are available : Description and Interaction ID.

It would be useful to add other fields to be populated by end-user.
