Alert Error - Occuring on SLA Alerts

Hi Experts,

So I am running into a weird issue on HPSM version 9.40 P2:

When any ticket breaches it's SLA, the Alerts won't send the email alert anymore. The following errors are shown in msglog:

15/04/17 11:58:14 - Unrecoverable error in application:  alert.process on panel notify.1

I added my own HTMLTemplates to the Notifications (SLA_Alert1 and SLA_Breach1) alongside the OOB alerts, but now neither work. Which looks like the following:
