Hi, when I applied the custom migration patch, the label reappeared, however now on another client the same thing happens.
Once the OOB migration patch is applied; the Attachment tab is not displayed.

According to my investigations the caption is based to the $attachmentCaption variable that is filled by the SYSATTACHMENTSService and AttachmentUtil ScriptLibraries using the function countAttachments.

For the Change management module this is done by the RuleSet chm.attachment.init.vars using the Initialize Attachment Tab Caption Rule.

This Rule is setup in the cm3r Object like in the attached screenshot

Please check if your system use correctly these elements.
