Before launching HP ALM Excel add-in select all the cells which contain data including headers. And try to upload.
Let me know if it works.

1. Try to check the data inside of cells. If there are write exact text "Subject..." (I mean to check if there are not some not allowed chars) and so on.
2. Try first export one test to excel so you will see the data.
3. Try to create new blank excel, put one row with simple data and import, like:
Subject Test Name
Subject TestA
and so on.
4. Try to import test to a blank QC project without mandatory user fields like Test Title ...

According your advice, re-do this action, still get the same error. 😞
Besides, I notice that “Type” in ALM Excel Add-in is not mandatory filed, while it must be set value into QC. Is this inconsistence the problem?

Hm, maybe.
But I think I am now stuck.
I would also check if this is a problem only on your computer or not. If yes then maybe there is wrong installation of QC or Excel addin.

2. Do not select headers while exporting
But still you shouldn't be getting 'value is required'.
Attach the Excel sheet..
