i am not able to create new folder under subject into test plan.
any idea ?

Strange...

Try:
1. refresh
2. Logoff / logon to QC
3. Logoff / close browser / logon to QC
4. Logon with another QC user
5. Reboot the server

when I see such lenghty errors...

I usually restart QC servce & DB service..

If both are on same case, you can reboot the machine once and give it a try.

Have you tried creating folders with different names? Maybe the name of the folder that you are giving already existing but is hidden...... Just a guess, but give it a try...

no, only deleted many records under audit tables (manualy from HPQC Admin interface)

3. 26 cubed = 17576 per level, (255-6)/3 = 83 levels (I believe, havnt tried)

Inspecting this table might help:
