Smartphone QR scan code fails

I'm trying to add a smartphone autentication method.
The problem is that the QR code scan fails.
The error message is:

I checked on both Android and iPhone phones.

Can you please explain what should I put on the Server URL parameter in the smartphone definition?

The “Device add error” can be found here:

So a solution here is the following: Administrator should verify correctness of Smartphone method configuration. This may be related to a conflict on IP address/port.

Make sure that OTP method is enabled and the proper setting for QR code type is in effect. You can select to use either NetIQ app or Google app.

If the incorrect one is selected then you should see a red indicator when trying to scan the code with the mobile app.
