UCMDB 10.11 and Active Directory Authentication Error

I'm trying to get UCMDB 10.11 enabled to allow LDAP Active Directory authentication.  After configuring the LDAP sections under Infrastructure Settings, I am able to see my LDAP Mappings in the UCMDB.  I'm also able to click the "Remote Group Users" button on one of the LDAP Repository groups and it lists the group members from Active Directory correctly.

I also have the option "Remote users repository mode" set to "True".

But when I try to logon to the UCMDB, I get the error "Authentication Failed".

I've attached a section of our bsf_security.log file showing the events at the time of a logon failure.

What might I be missing?

Thanks.
