Cannot unload medium, target slot appears to be occupied

HP Data Protector 6.11, when the backup job is finished and it tries to unload the medium, receive error "Cannot unload medium, target slot (xx) appears to be occupied. Backup statistics ahow as 1 completed media agent and 1 failed media agent. This error started a few days agao. Happens on all backup jobsa, different tape drives and different slot numbers. Any suggestions as to why this is happening?

Usually DP doesn't lose track of which tape was taken from which slot and put back in exactly the same slot it was taken from. Could be something to do with library settings maybe:

Is that the only error message? What happens after the message, where does the tape go?

Is it really occupied or just reported as occupied?

I beleive it is just reporting it as occupied. The backups seem to work but the sessions all shows green check marks with red X's.

Turns out the error message were reporting correctly, the cartirdges were full, someone from another site loaded it with one too many tapes and when he tape ejected or tried to eject , the slot was indeed full.
