Where do i get the user name so that I can use Fortify on Demand intellij plugin

On Fortify on Demand trial version,

Where can I get username, password, API root url for FOD Intellij Plugin?

I do have tenant ID, 

API root URL : https://api.ams.fortify.com

User Name tried as email address.

But still failed to login.

Able to login on Fortify account portal in browser.

Please advise.
