Comment system issues

I'm trying to review comments. There is no link to or mention of the original post so I have no idea what they are commenting on. 

In other cases they are asking a question, I definitely need a link to the post so I can answer.

Finally, when I click on APPROVE nothing seems to happen. Maybe it shows up then but no link to post so I can't tell.
