Enhance QA threshold incidents to display the measured value and the threshold value

Today I can get an QA incident like this:

QA Probe <Probe name> Echo has returned an error. Reason: Oper state is OverThreshold.

Neither the Incident message nor the Custom Attributes shows the measured value or the threshold value. This makes it harder to evaluate the severity of the incident. So the idea is to make these values visible.

I know it is possible to open the probe and look at the "Latest Polled Values", but it is a few steps to get there. It should be easy to place the value in the incident as well.

This idea may efect other threshold incidents, but my experience is this particular QA incident.
