capitalizing files by itself

Hi all: I have an OES2 sp3 server that for some reason will periodically change the name of a file stored on an NSS volume to all caps. Anyone seen this behavior? How do I stop it? Something in the back of my head says I have seen this before.

Is this server fully patched with latest patches installed?
