Change hostname and IP address on agent node should update OMi RTSM correctly

If you change both the IP address and hostname of a monitored node which has an agent, the ASSD info is sent to OMi containing the new hostname and IP address, but it causes duplicate topology to be created.  Ie, a new node CI, with IP address, interface and agent.

Request is to update the existing CIs rather than create new ones and remove the no longer used IP address.  Same goes for when MAC addresses change.  The node reconciliation rule can be modified to handle the reconciliation, but it does not clean up any no longer used related CIs (old IP and old interface).
