Microsoft SQL"SELECT" Query returns same row twice

I am using a Microsoft SQL/SQL Query operation to perform a SELECT operation. I have the "more items" reponse directing to a List Appender and then returning back to the Query operation. So if the SELECT operation has multiple rows it creates a list.

My problem is that the current query I am testing is only supposed to return one row, but it is going through the 'more items' loop twice and returning the same row twice before going to "no more items".  Does anybody know why this is happening or how to fix this?
