How do you identify dangers and opportunites for your company? Is it just
in your head? Or do you seek out input from your staff, underlings or even
overlings?

We wait until something bad happens and then say "Yep. That was a
danger."

Given any org chart...

dangers are at the top, opportunities at the bottom.

How to decide what's a threat or whatnot?
