Requests taking different workflow step intermittently

Hi Experts,

I am having the following Issue.

In HP-PPM we created a new form by adding two fields to the existing form. Now when the Users are raising the request using that form, intermittently it is going to one step in the workflow which otherwise it should skip.

For Example

Some requests are going for the Approval of System Admin at one step which should not happen. This issue is occuring with 3 out of 10 requests.

Kindly suggest how to resolve this Issue

Hello there,

Possibly there is a router in your workflow, that is diverting your requests based on the value selected on the form. 

Can you check your workfow? check the execution steps.

it would also be easier to debug if you look at the graphical view of the requests that are in question.
