Customize SRC form

How can we add new field value on SRC form. Looking for quick response.

Yes, you can update the checkout panels for  both Support and Service requests to include additional fields. The SRC Customization Guide outlines the procedure: the 9.40 version is attached, but you should download the correct doc for your current release. 

 

For example, after configuring the panel to prompt for email and phone number:

Thanks for the reply, SRC version we are using 9.33... We have requirement to have same information in one field
