SM-RC not syncing

We have SM7.11 RC9.13 and uCMDB 9.05. Since last two months, we have noticed SM data not synchronizing with RC properly.

Operation team able to see the CRs in RC application but Change 'Phase' and 'Phase Status’ showing in RC is NOT correct.

RC restart resolve the problem but this problem is repeating every day. Expecting permanent solution from the community.

Nothing has changed at SM and RC side.
