Upgrade 9.34 to 9.41 failed

Hi Experts,

we try to upgrade our development system (it is a fresh copy from production) to sm 9.41
We finished our last upgrade from 9.34 with PD3 to SM 9.40 with errors in October 2015.

The application version is still 9.34 , even we have 31 rad applications with the version number 9.40,
RTE Version is 9.41P3 , Webtier Version is 9.41

We using the upgrade procedure: KM01824178/SM930PD_SM941_upgrade.pdf
login (Client 9.41 P3 installed on sm server, Client side load/unload unchecked ) with an admin account, language english - dateformat mm/dd/yyyy
Steps:
*aapm.upgrade.purge
I’m done, and I want to remove the upgrade files completely
- finished purging upgrade file

load D:\HP\Service Manager 9.30\Upgradefiles\SM7.10-9.41.0020_L10N\preupg.bin
No errors:
purging a second time , because we following the Notes in the upgrade document (Step 4.5 )
*aapm.upgrade.purge
I’m done, and I want to remove the upgrade files completely
- finished purging upgrade file
Error:

Hi Experts,

maybe our steps helps another customer.

Step 1 : we imported the modified  Scriptlibrary idolUpgrade (from an other SM 9.41 System)  (239 lines and corrected 3 lines: 149, 150, 151)

Step 2: increased the lenght for exceeds fields in the tables: imCategory, format, globallist, displayoption

Step 3 : load preupg.bin

Step 4: analyses the necessary manual dbdict changes.

Step 5: load transfer.bin

Step 6: Use cmupgrade and Select Step 3 'configure and apply the Upgrade

The upgrade procedure could be proceeded without an error.

Hi Experts,

added the results from the next upgrade try

Step 1: We restored our DB

we changed the operator account to : language english, date format mm/dd/yyyy, currency Dollar

Step 2:

*aapm.upgrade.purge
I’m done, and I want to remove the upgrade files completely
- finished purging upgrade file

Error Messages appear that the process can not write to x:\Deployment/Release ..... (look at the screenshot)

we go forward, because we think we can ignore this problem

Step 3:

load D:\HP\Service Manager 9.30\Upgradefiles\SM7.10-9.41.0020_L10N\preupg.bin with Unload Manager

Step 4:

smupgrade

Load Upgrade Utility File - transfer.bin

Step 4:

smupgrade:

run SQL Compare Utility

Now we have to apply the manuel dbdict modification.

Hi Experts,

we fínshed the dbdict changes.

Unfortunately the upgrade failed after 74 % at chapter 4 - step 6 'configure and apply the upgrade'

with the following: errors:

Unrecoverable error in application: sm.upgrade.ux on panel.call.apply.upgrade.

Any ideas are welcome.

Hi Experts,

1, we changed the R-IT SL upgradeconvert and remarked the line 68: (calling function  lib.idolUpgrade.upgrade)

2.increased the lenght for exceeds fields in the tables: imCategory, format, globallist, displayoption

3, Upgrade is now finished with the error messages:

cancelling dbdict update, failed to execute the following SQL: ( screenshot)

Any ideas are welcome.
