Where are forms defined in menu options?

Hello experts,

We are trying to add another find a request page to our ess menu, one will search the tickets of the user signed in and the other searches the tickets for the county. Both work how we'd like right now, but they are both pulling from the same form (ess.sd.search). 

We'd like it so they each pull from two separate froms, but I can't find where to plug in the new form in the menu. I've attached a snip of our menu.

Hello Wbh13,

You can customize search form per each file.
for example,
db > Object > 'probsummary' then, you can can see "Search state:im.search"

In im.search, you can customize search form name.
