ANSWER
----------------------------------

First make sure you have added the executable to both the extension enabler and under options>Extensions and that you have the Activex option checked for both.

Also, if the executable name for your VB app contains spaces, make sure that the name, when added to the extensions, is enclosed in quotation marks.
