Monitoring MS SQL Servers with SiteScope

We are being asked to monitor MS SQL Server with SiteScope.  One of the things that the SQL Team is used (they were using MS SCOM) is the monitoring knowing dynamically all of the Database names, and being able to show them the status of each individual DB on their SQL Server.  I have not been able to locate a way to do this with SQL Server, and was hoping someone else has found it or written a query that can do that?

I have also tried the SiteSQL Solution Template, but that does not do it either.

SQL monitor monitors by instance not by DB. So you may use DB query monitor: http://msdn.microsoft.com/en-us/library/ms178534.aspx
